Volunteer Application
Thank you for your interest to volunteering at the Allen Ministerial Alliance Food Pantry.
Because St. Jude Catholic Parish has graciously donated the land on which the Food Pantry resides we must adhere to guidelines from the Catholic Diocese and our own in providing all our volunteers with a Safe Environment in which to serve.
All volunteers must clear a background check and complete a Safe Environment Course provided through the Dallas Catholic Diocese.
If you are already a member of St. Jude or one of the surrounding Catholic parishes you do not need to provide your Driver’s License # or the last 4 of your SS# as you parish will be responsible for performing these background checks once you’ve completed their Safe Environment course. Please enter 0’s for these fields.
Once this form has been submitted please allow our administrators to review your application before notifying you of next steps.

- I understand that all criminal background checks will be treated as confidential.
- I understand and authorize the access to any and all information and records relating to my criminal history or criminal offenses committed or alleged arrests, alleged criminal acts and criminal offenses committed.
- I am aware that background checks may be updated periodically.
- I intend this to be a legally binding release, which I have read and understand. I understand that I may consult with an attorney before signing this document. A facsimile or photocopy of this authorization shall be as valid as the original.
- If a disqualifying offense is found on a criminal background check, there is an appeal process. I understand that this process allows me to verify information and correct any errors.
